Why are readmissions a challenge? Hospital readmissions continue to pose quality and cost problems for the healthcare system. Although there has been some success and a slight reduction of readmission rates in recent years 1 , it remains a substantial problem in most healthcare populations. Current solutions are siloed and do not properly utilize analytics and technology. Care transitions programs utilize expensive staff and are challenging to scale.
